Wellness Benefits Included in Medical Trust Plans

As part of the Medical Trust's commitment to the overall wellness of our members, vision, mental health/substance abuse, employee assistance and health advocacy are offered as an integral part of each Medical Trust medical plan at no additional cost:

Vision Benefits
Our vision care benefit through the EyeMed network includes an annual eye examination with $0 network co-pay, prescription eyewear, and contact lenses offered through a broad-based network of ophthalmologists, optometrists, and opticians at both retail chains and independent provider locations. Mental Health/Substance Abuse Benefits

Our mental health/substance abuse benefits (MH/SA) are administered by Cigna Behavioral Health (CBH)*. Through our partnership with CBH, your employees will have access to an integrated behavioral health program that includes mental health, substance abuse, and employee assistance program benefits. This plan also offers coverage for clergy colleague group facilitators.

For the non-high deductible health plans, MH/SA services are not subject to the deductible, either for network or non-network providers. Network outpatient services require a $20 copayment across all of our plans (except the HDHPs). Our "network only" plans also feature a non-network MH/SA benefit for both inpatient and outpatient services.

*Due to IRS regulations, the mental health/substance abuse benefit for the Empire BCBS HDHP/HSA plans are administered by Empire BCBS as part of the medical plan. The benefit is a coinsurance after the deductible is met.

Employee Assistance Program (EAP)

Our Employee Assistance Program, managed by Cigna Behavioral Health, covers an array of services and assists our members and their covered dependents with information, educational materials, resources, referrals, and ongoing support. EAP services are free, confidential and available 24 hours a day, 7 days a week, through the CBH web site or by phone. The plan covers unlimited telephonic consultations and up to 10 face-to-face counseling sessions per issue with no copay. Cigna's EAP licensed clinicians can assist with problems including coping with stress, elder care, legal, financial, substance abuse, and emotional and physical health. EAP services also include immediate help during a crisis, local resources in your community on a wide range of topics, and tips and guidance to help balance work with family life.

Health Advocate
Our Health Advocate program is designed to help members navigate and handle medical and administrative issues within the healthcare system. Eligible members, their spouses, covered dependent children, parents, and parents-in-law are covered by this service. Personal Health Advocates, typically registered nurses, backed up by a team of subject matter experts, help members with resolving claim or billing issues, finding doctors and hospitals, assisting with elder care services, and scheduling appointments with hard-to-reach specialists.
